1) an application stating the applicant's name, state registration or registration number, phone number, email address

2) natural, processed or unprocessed, but unframed or loose, falling under the codes of the commodity list of the foreign economic activity classifier
a copy of the diamond trade, import, export, transportation license, if the import or export is carried out under the specified codes

3) copies of the contract and/or invoice, and in the absence of a contract, another document certifying the agreement between the parties

4) A copy of the Kimberley process certificate, if the import or export is carried out under the codes 710210000, 710221000, 710231000
